We are dedicated to helping people function better in their daily life at home, work, recreation and in the community.  Using safe and effective approaches, our multidisciplinary team addresses the needs of patients with acute and chronic orthopaedic, post-operative, multiple trauma, neurological and complex medical diagnoses.  After thorough evaluations, we develop individualized treatment plans with patient-centered goals that may include both in-clinic and at-home programs for exercise and learning recovery skills. 

How Patients are Referred to Us
Outpatient referrals are initiated by either the primary care physician or physician specialist.  Any required authorization for therapy is first obtained by the referring physician's office.  The referral, physician's orders for therapy, insurance authorization and relevant healthcare information are then faxed to our department.  Our department admissions specialist will then verify the insurance authorization before scheduling patients for their first visit.  Services can also be provided according to UPHH policy for a self-pay option.  The initial visit will most often include an interview and evaluation to determine the course of treatment.  The therapist will then submit requests for authorization of continued visits depending on the patient's insurance benefit requirements and level of progress.  Referrals can be faxed to 874-4291.

• All three disciplines provide site supervision for graduate and assistant level PT, OT and SLP students who would like to persue a career in rehabilitation.
