All information contained in the patient medical record is strictly confidential and is protected under the Health Insurance Portability and Accountability Act of 1996 (HIPAA).

Medical Records will not be released without written authorization of the patient, legal representation, a subpoena or court order.

• Records will be mailed to the requestor unless otherwise specified. 
• You must provide a photo ID before records are released.
• Fees may apply:
  • 1-5 pages:  No charge
  • 6 pages and over:  50 cents per page
