Postgraduate Training Registration (Arizona Medical Board)

Below you will find a link to the Postgraduate Training Registration form.

This form must be completed and accepted by the Arizona Medical Board before your start date with the University of Arizona or you will not be able to clinically see patients or receive wage benefits.

You do not have to send $50.00 as the form instructs. The GME Office pays the registration fees for all residents. You should complete the form as follows:

1. Complete all appropriate sections thoroughly (1-6)
